2010 6th Annual Cutting Edge Concepts in Orthopaedics & Sports Medicine Seminar

Course Description

This program focuses on the latest technology and surgical procedures used to treat orthopaedic conditions and sports medicine injuries associated with the shoulder, hip, knee and ankle as well as the use of manual medicine. A brief overview of the related anatomy as well as rehabilitation implications will be covered. Plus some common illnesses, syndromes, and/or conditions will also be discussed.

Registration

Deadline for registration is Monday February 1, 2010 (mail in registration must be received on by this date) Both On-Line and Mail-In registration starts on December 1, 2009.

Mail In Registration

Completely fill out registration form, then print out registration form, make a check out to Orlando Orthopaedic Center and send both registration form and check to: Bob Hammons, M.Ed., ATC, LAT, Orlando Orthopaedic Center, 100 W. Gore Street Suite 500, Orlando FL 32806.

Online Registration

Completely fill out the entire form and follow instructions for payment and or adding another attendee. You will be able to print out a receipt for your records. If you did mail in registration you may ask for a receipt at the On Site Registration Table during the registration process. Please check and make sure all registration information is correct before either mailing your form in or clicking complete registration for on line.

Confirmation/X-Press Check In Post Card

Mail-In Registration

Once your registration process is completed you will receive either an Orange X-press Check In post card or a Yellow Missing Information post card in the mail (with-in) a few weeks before the seminar. PLEASE bring this post card with you to the seminar. We use this card to stream line our registration process. This card will also have important information on it you will need to review before coming to the seminar. If you do not receive this card by Friday February 5, 2010 or forget to bring this card with you to the seminar you will have to go through the On Site Registration Line to check in. If you receive a yellow confirmation post card, your registration is incomplete and missing information Missing information must be given at the On-Site Registration table before your registration will be complete and able to check in.

On-Line Registration

Once your registration process is completed you have the option to print out a receipt. Fee

A $30 early registration fee is for the January 29th for Mail -In or February 2nd for On-line. The On-site registration fee is $45 the day of event. All registration fees seminar program cost, continuing education credits and lunch. All proceeds will benefit the UCF Student Athletic Trainers’ Organization. Please make checks payable to Orlando Orthopaedic Center.

Refund Policy

A written request for cancellation and refund will be granted prior to January 22, 2010. No refund will be made after January 22, 2010.

Lunch

A light lunch and a drink are included with your registration fee. An alternative lunch entree may be substituted for a nominal fee. When registering, please indicate your lunch choice. If not indicated on your registration, you will receive a cheese pizza.

  • Personal Pan Pizza – Cheese or Pepperoni (included in registration fee)
  • Grilled Chicken Caesar Salad (substitute for additional $5)
  • Garden Salad (substitute for additional $5)
  • Turkey Sandwich (substitute for additional $5)

Super Bowl Mania Contest Giveaway

The first 100 registrants are automatically entered in the Super Bowl Mania Contest Giveaway. You must be registered to participate. Some prizes include, but are not limited to, a complimentary golf foursome to Champions Gate, an Wii game system and a Garmin GPS. Prizes will be announced at the seminar and are subject to change. Winners will be determined on Super Bowl Sunday, February 7, 2010.
